KC Lupp and this is Tulsa Daily. You may have some back problems and you may not know if it’s sciatica or not. Well, helping us out on Tulsa Daily and Fox                                         23 today is Dr. Kris Parchuri of Spine & Orthopedic Specialists. Sciatica, I hear, and I’m not really sure exactly what it is. Is it just a low back situation? What                                           are the symptoms?

Dr. Parchuri:

Well, sciatica is a term utilized when there’s compression of a nerve coming off the spinal cord. So whether it’s from a bone spur or whether it’s from a disc, and                                       there’s pressure or inflammation, irritation on a specific nerve as it comes off the spinal cord, and it associates symptoms can cause pain, numbness and tingling down the arms or down the legs or into the chest wall and cavity.

Host:

What about massage? Does massage help?

Dr. Parchuri:

Massage can help sometimes. Unfortunately it can also irritate, but it can definitely help in some situations.

Host:

If somebody’s feeling that tingling or the numbness maybe down the leg, what do they do? They come see you and get diagnosed, and then what’s the next step?

Dr. Parchuri:

Well fortunately, most issues associated with pressure on a spinal nerve improve on their own. When utilized just rest, anti-inflammatories, ice, those kind of things, those symptoms typically tend to resolve. When they don’t and they’re persistent for more than 2 to 3 to maybe jsometimes 6 weeks, or you have associated weakness with the muscles, then you need to seek immediate medical attention.

Host:

Now if it does not alleviate within the 6 weeks or so and they come see you, surgery is an option.

Dr. Parchuri:

Well, no. You don’t typically jump into surgery fast like that.

Host:

Good.

Dr. Parchuri:

If someone has what we call foot drop, such as they can’t walk or they can’t use their arm, then you need to consider surgery because it’s obviously more than just a simple irritation of a nerve. The first thing that you want to do is obtain imaging studies, MRI’s typically the goal standard. You identify the exact reason why they’re having the symptoms. Then you can identify a treatment plan.
